The Nurse Practitioner can see patients for the following reasons; acutely unwell patients aged 16 years and over with minor illnesses and injuries, complex chronic disease management, sexual health and contraception including implants, minor surgeries, women's health and skin conditions .

The Practice Nurses (supported by the Healthcare Assistant) may be seen by making an appointment. They see patients for annual chronic disease reviews including visiting our Housebound patients, contraception reviews, well women's clinics including smears and HRT reviews, would management, baby clinics, health and travel vaccinations and blood pressure checks.

District Nurses

Referrals will be made to the District Nurse for those patients who are Housebound or temporarily housebound to carry out procedures including blood pressure checks, blood tests as requested by our clinical team, wound management and health vaccinations.
